For the question of what the nearest office will be, please select the STS branch office you are closest to, or the office you are interested in working with. ‘Fridley’ and ‘Minnetonka’ are in the Minneapolis, Minnesota area. ‘Las Vegas’ and ‘Reno’ are in Nevada, and ‘Phoenix’ is in Arizona. Mount Juliet’ is in the Nashville, Tennessee area.

You may enroll or decline any of the medical insurance plans. If you are not sure at the time of this application process whether or not you want to elect coverage, you can decline now and make changes within the next 30 days. If you do not make any changes within the 30 days you will have to wait until the next open enrollment period.
